  RATES and INSURANCE
Rates:
- The fee will be mutually agreed upon at the time of the 1st session
- A Reduced fee for services are available on a
limited basis
- Individuals: 50 minute in depth counseling session addressing your needs and concerns
- Couples: 75 minute in depth counseling session tailored to the needs of the relationships [prorated accordingly]
- Payment in full at the time of service by cash or check, unless otherwise arranged
Insurance: - I will provide you with a superbill which contains all of the provider information required by your insurance company upon request for you to submit
- Clients with the following insurance coverage will pay a co-pay and billing will be done accordingly through my office for the following insurance carriers: Blue Shield, Magellan, Cigna, MHN, APS, Carebridge EAP, Coventry 1st Health, Multi-Plans/PHCS, a/o April 30, 2011
